OZEMPIC® 0.25mg

Ozempic® is an injectable prescription medicine indicated for the treatment of adults with insufficiently controlled type 2 diabetes mellitus.

• An adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
• To reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes mellitus and established cardiovascular disease.

The active ingredient in Ozempic® is semaglutide, and inactive ingredients include disodium phosphate dihydrate, propylene glycol, phenol, hydrochloric acid, sodium hydroxide, and water for injections.

Follow the recommended dosage and administration instructions provided by your healthcare provider.

Contraindications:

• Personal or family history of medullary thyroid carcinoma (MTC) or in patients with MEN 2.
• Serious hypersensitivity reaction to semaglutide or any of the excipients in Ozempic®.
• Discontinue Ozempic® in women at least 2 months before a planned pregnancy due to the long washout period for semaglutide.

After-care:

• Store your pen in use for 56 days at room temperature or in a refrigerator.
• Use Ozempic® 1 time each week, on the same day each week, at any time of the day.
• If you miss a dose of Ozempic®, take the missed dose as soon as possible within 5 days after the missed dose.
• Stay on your prescribed diet and exercise program while using Ozempic®.
• Talk to your healthcare provider about how to prevent, recognize, and manage low blood sugar (hypoglycemia), high blood sugar (hyperglycemia), and problems you have because of your diabetes.
• Regularly monitor your blood sugar levels and hemoglobin A1C as directed by your healthcare provider.
• Do not share your Ozempic® pen with other people.
