MOUNJARO® 5mg KwikPen® is given as a once-weekly subcutaneous injection, at any time of day, with or without food. Suitable sites include the abdomen, thigh, or back of the upper arm; the upper arm is appropriate when a caregiver or clinician administers the dose. Injection sites should be rotated with each weekly dose to minimize local reactions.

The standard titration schedule begins at 2.5mg once weekly for 4 weeks, then advances to 5mg once weekly. The 5mg dose is one of three recognized maintenance levels — alongside 10mg and 15mg — and patients may continue at this strength when response and tolerability support it. Subsequent dose increases, if needed, are made in 2.5mg increments at intervals of at least 4 weeks, up to a maximum of 15mg once weekly.

If a dose is missed, administer it within 4 days (96 hours) of the scheduled dose. If more than 4 days have passed, skip the missed dose and resume the regular weekly schedule. When insulin is co-administered, inject at a separate site and do not combine products in the same injection. For information on other MOUNJARO® KwikPen® doses across the titration range, contact the Med Supply Solutions team.

MOUNJARO® contains tirzepatide and is FDA-approved as an adjunct to diet and exercise to improve glycemic control in adults and pediatric patients aged 10 years and older with type 2 diabetes mellitus. Within the Weight Loss & Metabolic category, tirzepatide’s dual receptor activity makes it relevant to metabolic and weight-management protocols; brand-specific indication language should nonetheless be interpreted in accordance with current local labeling and the approved prescribing information.

Tirzepatide serves as the active ingredient in both MOUNJARO® (indicated for type 2 diabetes) and Zepbound® (indicated for chronic weight management). The distinction between these brands is significant for prescribing decisions, procurement planning, and jurisdictional compliance, as approved indications can differ across regions.

MOUNJARO® is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC), in those with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), and in patients with known serious hypersensitivity to tirzepatide or any excipient. Safety and efficacy have not been established in pediatric patients under 10 years of age for type 2 diabetes or under 18 years of age for weight management.

Each dose of MOUNJARO® 5mg KwikPen® delivers tirzepatide 5mg in 0.6 mL. Tirzepatide is a dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist whose combined receptor activity sets it apart from GLP-1-only therapies used in metabolic care.

The mechanism involves enhanced glucose-dependent insulin secretion, suppression of glucagon, slowed gastric emptying, and reduced appetite and energy intake — a profile that reflects the additive contribution of GIP receptor engagement alongside GLP-1 activity. The solution is clear and colorless to slightly yellow, supplied in a single-patient-use KwikPen® multi-dose pen with a total fill volume of 2.4 mL, delivering 4 doses of 0.6 mL each.

Excipients include benzyl alcohol, glycerin, phenol, and further ingredients detailed in the prescribing information; the benzyl alcohol content is relevant to the multi-dose pen format. Refrigerate at 2–8°C (36–46°F) and protect from light. The pen may be held at room temperature up to 30°C (86°F) for no more than 21 days. Do not freeze.

For clinical context: the SURMOUNT-1 trial reported an average body weight reduction of approximately 21% at 72 weeks with the 15mg tirzepatide maintenance dose. This result is specific to the 15mg dose and does not apply to the 5mg maintenance dose.

The MOUNJARO® KwikPen® is a pre-filled, dial-free, fixed-dose multi-dose pen that delivers 4 once-weekly subcutaneous injections per pen. Fit a new needle before each use and do not reuse needles between doses. Before each injection, visually inspect the solution — it should appear clear and colorless to slightly yellow. If the solution is cloudy, discolored, or contains visible particles, do not use the pen.

Administer subcutaneously into the abdomen, thigh, or back of the upper arm, rotating the injection site at each weekly dose. After administering, remove the needle and immediately discard it in an approved sharps container. Prescribers, patients, and caregivers should consult the manufacturer’s Instructions for Use and the package leaflet prior to first administration.

Keep the pen refrigerated at 2–8°C when not in use. It may be stored at room temperature (up to 30°C) for up to 21 days. Protect from light and do not freeze at any point during storage.
