At 15 mg, patients have reached the maximum once-weekly dose in the tirzepatide prescribing schedule. No further escalation is available beyond this level, and patients established here may continue at 15 mg for as long as clinical response and tolerability support ongoing treatment. MOUNJARO® 15 mg KwikPen® is administered once weekly by subcutaneous injection, at any time of day, with or without food.

Injection sites are the abdomen, thigh, or — when another person administers the dose — the back of the upper arm. Sites should be rotated with each dose. For context, the full titration schedule begins at 2.5 mg once weekly for 4 weeks, then increases to 5 mg. If additional glycemic response is needed, the dose may be increased by 2.5 mg increments after at least 4 weeks on the current dose, up to this 15 mg maximum. Clinicians managing patients across the titration range may refer to other MOUNJARO® KwikPen® doses for full dose-range reference.

If a dose is missed, it may be administered within 4 days (96 hours) of the scheduled date. If more than 4 days have passed, skip the missed dose and resume the regular weekly schedule. When using MOUNJARO® with insulin, administer as separate injections and never mix. Both injections may be given in the same body region but not adjacent to each other.

MOUNJARO® (tirzepatide) is FDA-approved as an adjunct to diet and exercise to improve glycemic control in adults and pediatric patients aged 10 years and older with type 2 diabetes mellitus. In pediatric patients, the maximum recommended dose is 10 mg once weekly. Use should be interpreted in accordance with current local prescribing information.

MOUNJARO® is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), and in patients with a documented serious hypersensitivity reaction to tirzepatide or any excipient in the formulation. MOUNJARO® is not indicated for patients under 10 years of age.

Tirzepatide is the active ingredient in two separately approved products. MOUNJARO® carries the type 2 diabetes indication; Zepbound® carries a distinct indication for chronic weight management. The two products are not interchangeable.

MOUNJARO® 15 mg KwikPen® is supplied as 60 mg/2.4 mL, providing four 15 mg/0.6 mL doses per single-patient-use pen. In the U.S. prescribing information, 5 mg, 10 mg, and 15 mg are the available weekly dose strengths after titration; 15 mg is the maximum. The formulation is a clear, colorless-to-slightly-yellow sterile solution.

Tirzepatide is a dual GIP and GLP-1 receptor agonist that stimulates glucose-dependent insulin secretion, suppresses glucagon, delays gastric emptying, and reduces appetite and energy intake.

Excipients include benzyl alcohol, glycerin, and phenol, among others listed in the full prescribing information; benzyl alcohol content is relevant to the multi-dose pen format. Store refrigerated at 2–8°C (36–46°F) and protect from light. If removed from refrigeration, the pen may be kept at room temperature up to 30°C (86°F) for up to 21 days. Do not freeze.

In the SURMOUNT-1 trial, participants assigned to the 15 mg tirzepatide arm — an obesity study conducted in patients without type 2 diabetes — achieved a mean body weight reduction of approximately 21% at 72 weeks. This result was observed in that specific trial population and indication context; it does not represent a labeled outcome for MOUNJARO® in type 2 diabetes.

Attach a new needle to the KwikPen® immediately before each weekly dose — needles must not be reused between administrations. Before injecting, examine the solution: it should be clear and colorless to slightly yellow. If the solution is cloudy, discolored, or contains visible particles, do not use the pen.

Administer subcutaneously into the abdomen, thigh, or — when another person is administering — the back of the upper arm. Rotate the injection site with each weekly dose. After the injection is complete, remove the needle without recapping and dispose of it in an approved sharps container.

Prescribers and dispensing professionals should confirm that patients and caregivers have been trained on the manufacturer’s Instructions for Use and the package leaflet prior to first administration. Store the pen refrigerated at 2–8°C between uses. If removed from refrigeration, keep at room temperature up to 30°C for up to 21 days. Protect from light. Do not freeze.
