What Is ORENCIA® Used For? 

ORENCIA® prevents an autoinflammatory attack on the joints by binding to receptors on antigen-presenting cells (APCs) and preventing the activation of T cells. It’s used for a range of autoimmune disorders, including rheumatoid arthritis, polyarticular juvenile idiopathic arthritis, and psoriatic arthritis.

Rheumatoid Arthritis

Rheumatoid arthritis is a chronic disease that affects the joints, heart, lungs, eyes, and skin. ORENCIA® is approved for use in moderate to severe rheumatoid arthritis in adults (18 years or above). 

Polyarticular Juvenile Idiopathic Arthritis (pJIA)

pJIA is diagnosed when arthritis occurs in an individual who is 16 years or younger, with the symptoms lasting at least six weeks. pJIA occurs due to dysregulated T-cells, which makes ORENCIA® an effective treatment method as it stops the activation of these cells. ORENCIA® is approved for pJIA patients who are two years or older. It can be used alone or alongside methotrexate. 

Psoriatic Arthritis

Some patients with psoriasis develop arthritis, which most commonly affects the fingers and nails. ORENCIA® is approved for psoriatic arthritis patients who are two years or older. In children, it may be used alongside methotrexate or alone. In adults, it can be used alongside non-biological treatments or alone. 

ORENCIA® Administration Technique 

ORENCIA® is available in two forms: intravenous and subcutaneous injections. 

Intravenous Infusion

The intravenous form takes around 20-30 minutes to administer, though the actual time varies depending on the clinic setting. It is initially administered on week zero, week two, and week four. After that, it is administered once every four weeks.   The intravenous form is approved for those six years of age or older—it is not suitable for patients under six years. It is also not suitable for psoriatic arthritis patients of any age. 

Subcutaneous Injection 

The subcutaneous form of ORENCIA® is available as two syringes—as a prefilled syringe or as a ClickJect™ Autoinjector. It is administered once every week.  Because a subcutaneous injection does not demand the setup necessary for an intravenous dose, patients can choose to inject this form of ORENCIA® themselves. However, they must first be trained by a physician to ensure proper administration. 

ORENCIA® Side Effects 

Common ORENCIA® side effects include:
  • Headache
  • Sore throat
  • Nausea
More severe side effects include:
  • Infections: ORENCIA® increases susceptibility to infections because it disrupts the activation of T cells. This leaves the immune system incapable of mounting an effective response against ongoing or new infections. Signs of a serious infection include a high fever, cough, or flu-like symptoms.
  • Allergies: Some individuals may be allergic to abatacept. The severity of an allergic reaction can vary. In severe cases, it can manifest as swollen lips, eyelids, tongue, and face, as well as hives and difficulty breathing.
  • Hepatitis B: Individuals who have been diagnosed with hepatitis B in the past require extra caution before starting treatment with abatacept. Abatacept’s interaction with the immune system can activate a latent hepatitis B infection. Therefore, disclosure of a past hepatitis B infection and testing for one if there is no past history is essential.  
  • Breathing problems: Breathing problems can occur in individuals who have chronic obstructive pulmonary disease. They require immediate medical attention.
  • Cancer(s): Although a clear relationship between abatacept and the risk of cancer has not yet been established, there are multiple reports of such an association. For example, this French observational study suggests that abatacept increases the risk of melanoma.

ORENCIA® vs. The Competition 

There are multiple drugs available as alternatives to ORENCIA®. Medical professionals usually choose between it and Remicade.
  • REMICADE: REMICADE contains infliximab, which is also a monoclonal antibody. A phase III trial compared infliximab and abatacept in rheumatoid arthritis patients. The trial found similar efficacy for the two drugs, although abatacept had a slightly better adverse event profile with fewer discontinuations.
